If a home that you visit has the home's listing agent on hand, and you think that you might like to make an offer on that home, stop!  Remember that the listing agent, and his firm, legally represent the seller, not you.  Who will represent YOU?  
 
No matter what story the listing agent tells you, you SHOULD HAVE your OWN buyer's agent to represent YOU and protect YOUR interests.  And it costs you no more to do so. 
 
Therefore, do this: just show the agent this form and say that you intend to be represented and protected during your purchase and escrow by your OWN agent.
